Intent of the Fund

• To assist with lost wages due to injury during a SVICSA game/practice (assuming no medical leave, sick day provision, EI assistance).
• To assist with medical costs (treatment, medications) not covered by personal insurance plans and/or league insurance plans.
• Financial need.

Fund Guidelines

• The team/club is expected (but not required) to match the SVICSA assistance.
• The maximum amount of SVICSA assistance for any one person (per incident) is $600.00
• The submission of the Application Form.
• An email from the players Manager in support of the application.

Important!

Claims against the league insurance policy must be made promptly. According to our Insurer's policy guidelines "written notice of injury on which claim may be based must be given to the Insurer within sixty days after the date of the accident causing such injury."
