The South Vancouver Island Classics' Soccer Association
Promoting togetherness on and off the pitch

 

    Team managers are requested to use the following form to communicate the misconduct of players, team officials, game officials or spectators. The contents of the form are automatically forwarded to the head of the league's disciplinary committee. Please fill out the form completely to ensure all the necessary information is communicated, allowing an independent panel the means to fairly assess the incident, and to penalize appropriately. Please use the following form within 24 hours of the incident in question.
